The STCW – Medical Care (EU) training prepares seafarers to provide effective medical care on board when access to shore-based facilities is limited. Participants learn life-saving techniques, how to make diagnoses, carry out treatments, and use the medical equipment available on board.

IntroductionOn board ships, seafarers often have no direct access to regular medical facilities. Therefore, designated crew members must be capable of providing medical assistance in the event of accidents or illnesses. The STCW Medical Care course focuses on performing medical procedures independently and applying the correct protocols in consultation with doctors from the Radio Medical Service. This training contributes to maintaining a high standard of medical care on board and ensures the safety and well-being of the crew.
ObjectiveUpon successful completion of the course, participants will be able to:
Act effectively in the event of accidents and illnesses on board
Conduct medical examinations and establish accurate diagnoses
Use medical equipment and medicines on board correctly
Communicate effectively with the Radio Medical Service for professional advice
Provide first aid and perform essential medical treatments
Target groupThis course is intended for captains and seafarers responsible for providing medical care on board vessels operating under the STCW Convention.
Entry requirementsTo participate in the Medical First Aid training, participants must hold:
A valid Basic Safety Training certificate.
To participate in the Medical Care training, participants must hold:
A valid Medical First Aid certificate.
Course contentThe course combines theoretical instruction with practical exercises and covers topics such as:
Basic resuscitation (Basic Life Support)
Use and management of medicines on board
Physical examination of the abdomen and chest
Treatment of wounds, fractures, and traumatic injuries
Suturing, administering injections, and intravenous infusions
Pathology and recognition of common illnesses
Hypothermia and other medical emergencies
Transport of injured persons on board
Communication and procedures with the Radio Medical Service
CertificationUpon successful completion, participants receive one or both of the following certificates:
Proficiency in Medical First Aid, STCW Reg. VI/4, Section A-VI/4, Table A-VI/4-1and/or
Proficiency in Medical Care, STCW Reg. VI/4, Section A-VI/4, Table A-VI/4-2
ValidityFive years.

Medical First Aid: 2 consecutive days
Medical Care: 3 consecutive days
Medical First Aid and Medical Care combined: 5 consecutive days
Number of participantsMinimum of 4 and maximum of 12 participants.
Course hours08:30 – 17:00 hours.
Course recognitionThis training is recognized by the Dutch government (Human Environment and Transport Inspectorate), the competent authorities of Belgium (EU), and the Marshall Islands.
Day one covers the fundamentals of pathology, including infectious diseases and physical examination techniques such as chest and abdominal assessment. Participants learn to recognize and evaluate various illnesses and practice administering injections and intravenous infusion
The second day focuses on general wound care, local anesthesia, and suturing. Participants practice wound-closing techniques and apply their skills in realistic training scenarios using lotus casualty actors.
The final day is dedicated entirely to hands-on medical scenarios with lotus casualties. Participants demonstrate their ability to diagnose, treat, and make medical decisions under realistic onboard emergency conditions.

Mathanje den Boer-Besemer is married and the mother of three wonderful boys. She was born in 1976 on a Dutch military base in Blomberg, Germany, where she spent most of her childhood. At an early age, she obtained all her swimming diplomas and immediately joined the DLRG (Deutsche Lebens-Rettungs-Gesellschaft). Around the age of fourteen, she also began assisting with teaching the youngest children.
Shortly before her seventeenth birthday, she returned to the Netherlands. There, she joined a local lifeguard association and completed her MDGO-AB education. However, she did not fully find her passion in this line of work. She started a university of applied sciences program to become a recreation officer but soon discovered that she is more of a doer than a thinker who sits in an office. This led her to switch to swim instruction, where she specialized in adapted sports. Building on this, she co-founded a foundation for people with medical limitations together with a friend, which she led for 25 years.
In the meantime, it became increasingly clear that the field of emergency services continued to attract her. She joined the Red Cross, became an instructor for both the Orange Cross and the Red Cross, and after five years of teaching the crew of KNRM station Hansweert, she herself became a crew member ten years ago. Since then, she has gained international experience, including an exchange in Croatia and training in Finland with Women in SAR.
Also ten years ago, Mathanje started working at DRTC as a first aid instructor. She greatly enjoys teaching and values the contribution she can make in the world of emergency services—whether with the Red Cross, Witte Kruis, the riot police, or the KNRM. For her, it is meaningful and rewarding work, and she hopes this is also reflected in her lessons.
With over thirty years of experience as a hospital nurse, Iris Marico has gained extensive expertise across various medical specialties. She brings this wealth of knowledge and hands-on experience to De Ruyter Training & Consultancy (DRTC), where she provides Medical First Aid (MFA) and Medical Care (MC) training at the Vlissingen location.
Alongside her work at DRTC, Iris continues to practice in the hospital, ensuring she stays up to date with the latest medical developments, regulations, and protocols. She translates this current medical knowledge into practical applications relevant to the maritime industry.
With a partner working in the maritime sector, Iris also has a strong understanding of daily life at sea. This allows her to connect the STCW medical training standards to real-life scenarios, helping participants not only learn the required procedures but also understand how to act effectively in emergency situations.

Yes, our STCW courses are recognised by the Dutch flag state (the Human Environment and Transport Inspectorate – ILT). Each certificate includes the relevant STCW reference, ensuring international recognition.
We offer Basic Training (BT), Advanced Fire Fighting (AFF), Medical First Aid (MFA), Medical Care (MC), Proficiency in (Fast) Rescue Boats (PSCRB/FRB), and Security Awareness/Designated Duties.
Most STCW certificates must be renewed every 5 years (depending on the module and sea time requirements). Check the course page for exact details.
Yes, a valid maritime medical certificate is mandatory for participation.
Bring valid identification, a medical certificate (mandatory), and swimwear/towel if the course includes a pool module. PPE and rescue equipment are provided by DRTC.
Courses are available in Dutch or English. Additional support for reading difficulties can be arranged on request.

You can cancel your booking by contacting us via email or phone. For open enrolment courses you may cancel free of charge up to 5 working days before the start. For customised training you may cancel free of charge up to 14 working days before the start. If you cancel within those deadlines the full course fee will be invoiced.
